Patient Requirements Checklist
A summary of the requirements for a Patient receiving a test.
Before test appointment#
Log into your patient portal and fill out draft forms shared by the Office.
☐ Portal Account: For your first test, create your patient portal account from the invite the Office sent.
☐ Log In: Log into your patient portal at covidtesting.icehealthsystems.com 
☐ Test Form: Fill out the draft copy in your patient portal.
☐ Patient Insurance Form: Fill out the draft copy in your patient portal.
Repeat Tests
If you filled out a Patient Insurance Form before, the Office might not ask you to fill out a new copy.
