Fill out forms in your patient portal
To fill out forms shared with you by an office:
- Login to your patient portal at covidtesting.icehealthsystems.com
- Go to the Forms menu and then select any draft form(s) shared by the office
- Select Edit Form to fill out the details and Review Form when you're done
- Repeat for any additional draft forms shared from the office
- For example: A Patient Insurance form and then a Test Form.
- Logout of the portal when you're finished
Now the office has your completed forms and can review them with you during your next appointment.
If you forgot your username or need to reset your password, select Forgot username? or Forgot password? on the login screen and follow the prompts.
